The lease agreement worksheet builds the written agreement for one tenancy from the facts of that tenancy rather than from a blank template: the property and unit, the landlord and the tenants who will sign, the term and its start and end dates, the rent and the day it falls due, the deposit held, who pays which utility, whether pets are permitted and on what terms, and the addenda that ride along with it. It renders a lease you can print, sign and file, and it shows you which facts are still missing before it is a complete agreement. What it does not do is tell you what your state requires a lease to contain: that varies in every state and it is your attorney's work, not a worksheet's.

What do I need in front of me before I fill in the lease agreement? The address of the property and the unit, the legal names of the landlord and of every adult who will sign, the start and end dates of the term, the monthly rent and the day of the month it falls due, the deposit you are holding and any pet or parking terms you have agreed. If you do not have the deposit figure settled yet the worksheet will still render, and it will mark the line as outstanding rather than guessing.
Is this lease valid in my state? It is a plain written agreement covering the things a tenancy is actually made of, and it is not a state-specific form. Every state adds its own required disclosures and its own limits on deposits, notice and late fees, and several require particular wording. Have your attorney read it once against your state before you use it for real; after that the worksheet is doing the same job every month with different figures.
Can I add my own clauses and my own name to it? Yes. Addenda are part of the worksheet, so a pet addendum, a parking addendum or a utility agreement rides with the lease rather than being a loose page.
